Thanks for finding that article. I wonder if some of the equipment failures: No AIS use (situational awareness of the Navy Vessel/civilians--could use limited power to avoid advisories from knowing tactical positions.). Radars not working/ watch standers not recognizing what they see.

All of the accidents were preventable--and involved personal not doing their jobs in the best manor. This includes execs not stopping voyages where there is known danger if equipment is not functioning and personal frictions prevent smooth functioning.

The fatigue is something that we recreational voyagers experience when we cross oceans. But we adjust and keep our watches.
