Mike this is what I got from your instructions:
1. Highlight the word or phrase you want to be the placeholder for the link
2. and click the url button.
3. Then carefully position the cursor between the = and the ] in the first tag
4. and paste the link address right after the =. Again, no extra spaces.
BUT I don't see an equal sign (=) in the "url" tags.
