Tessa's V-berth does not have a cushion between the legs of the V, but it does have grooves where the cushion would appear to belong. Has anyone made an insert, and do you have plans, suggestions, warnings, photos, specifications?

There is normally an insert there. If you cannot find one many shops should be able to make one up.

My 25 came with the insert from the factory. Let me know if you need measurements or specific photos. It shouldn't be very difficult to replicate.

The factory insert is a cushion covered in material to match the berth cushions. It is mounted on a board which sits on those small wooden cleats that are probably mounted on the sides of the vee berth legs. In my opinion, the good is that the cushion gives you more sleeping area, the bad is it makes it more difficult for my wife and I to crawl in and out of the "cave". We find it nice to be able to sit down and swing our legs onto the cushions and have taken the insert off the boat.
